These centers offer clients a stimulating, homelike daytime environment, which also gives caregiving partners the respite they need to recharge. Community Centers for Seniors & Disabled Adults

OMI Senior Center
This multi-purpose community center brings seniors together for a hot nutritious lunch and social programs such as arts & crafts, singing, dancing, educational seminars, and day trips. The center also offers on-site case management services from qualified staff. Many times these services include counseling, dietary advice and relaxation techniques. "Since coming here, my self-esteem and energy have gone up," says one client. "Plus, I've met new friends—even at my age, believe it or not." The Center also includes an In-Home Support Services Program, which helps seniors with daily living chores—including laundry, house cleaning, shopping and personal grooming. The OMI Senior Center is a sensible way to help seniors stay physically and mentally active.

San Francisco & San Carlos Adult Day Services
Our licensed Adult Day Centers are home-like daytime facilities that offer frail elderly and disabled adults a safe, therapeutic and friendly environment. The programs are designed to enhance the participants’ quality of life and increase their independence. Participants enjoy a safe, fun and stimulating environment while families are eased of their constant responsibility of providing care. Frail seniors and disabled adults who experience losses in physical function, memory or social support my benefit from the health services, socialization, mental and physical stimulation, supervision and recreation offered.

  • Services We design a personalized plan of care to meet the unique needs of each participant and their caregiver, including assistance with activities of daily living, supportive services, caregiver education and ongoing support. Adult Day Services is a sensible and affordable alternative compared to in-home care or placement in a residential facility.
  • Activities Clients obtain an increased quality of life through a variety of stimulating and therapeutic activities including entertainment, art therapy, cooking group, bingo, daily exercise, mental stimulation groups, sport therapy games and holiday celebrations.
  • Transportation and Nutrition We coordinate assisted transportation through local paratransit to and from our Centers and provide breakfast, a hot nutritional lunch and snacks.

San Francisco Alzheimer's Day Care Resource Center.
"My wife has Alzheimer's," says one client's husband. "She'd be in a nursing home if it weren't for the Adult Day Center. During the day, I get to rest. And at night, I get to keep her with me." This Center, which serves moderate- to late-stage Alzheimer's patients, offers a hot nutritious lunch, two delicious snacks and a safe, caring environment that helps clients mingle, learn, have some fun, and remain stimulated, both mentally and physically.

Geriatric Care Management
Skilled, caring professionals provide care solutions for a range of situations. A team of licensed therapists, social workers, registered nurses and gerontologists work together to coordinate care, provide in-home assessments, advocate for elders throughout the medical community, design individualized care plans, and support and educate family members and caregivers.

Home Care
The philosophy behind this care is to help seniors live safe, healthy, independent lives in their own homes…for as long as possible. This program features a comprehensive range of highly-trained and credentialed caregivers, including Certified Nursing Assistants, Certified Home Health Aides, homemakers and personal care attendants. Daily living help includes assistance with bathing and dressing, light housekeeping, cooking, grocery shopping, laundry, medication reminders, errands, and escorts to appointments. Not to mention, the ability to just sit and listen.

Counseling
Counseling by licensed therapists or supervised interns is available to clients and their family members who find themselves in a variety of situations, including: depression, isolation, and overwhelm. Counselors are available for children, adults, couples, seniors, families and groups. Specialized grief counseling is another of the many services provided. English- and Spanish-speaking counselors are on staff.
