Housing, help & hope for those living with HIV. At the outbreak of the AIDS epidemic over 20 years ago, the Assisted Housing & Health Services of Catholic Charities CYO was one of the first organizations to respond. Since then, our services have grown and evolved to continue to meet the ever-changing face of the epidemic. Today, we serve more than 1,300 people living with HIV, annually. Our total number of clients over the years is 6,000—25% of all San Franciscans who've been disabled by the disease. "Catholic Charities CYO helped me choose life," says one client.

Permanent Supportive Housing

Peter Claver Community
This 32-bed licensed medical residential care community serves formerly homeless men and women who are living with disabling HIV/AIDS, many of whom also struggle with major psychiatric disorders and substance abuse.

Leland House
Leland House is a permanent/supportive housing program for 45 people living with disabling HIV/AIDS, psychiatric disorders and substance abuse issues. 10 beds of the 45 are available for those who require assisted living and/or assistance with activities of daily living and are homeless. Residents for these 10 beds can be referred directly and quickly from hospitals, shelters, SROs and agencies working with the homeless.

Leland House is a residential care community that provides nursing and social work case management as well as a staff of certified nursing assistants to help with activities of daily life, a full kitchen staff that provides three meals a day and an activities coordinator to help improve and sustain quality of life.

Independent Supportive Housing

Derek Silva Community
"At Derek Silva, I can live on my own," says one client. "There's nothing more important to me than my independence." This 68 unit building maintains onsite case management, an activities program and other supportive services to assist residents with a wide range of needs.

Housing Support & Case Management

Assisted Housing & Health Programs.
We help by providing more than 237,250 nights of housing annually through the full and partial rent subsidy programs. The program also provides case management, money management and housing advocacy services. For clients who also need support for medical expenses not covered by private insurance or Medicare/Medicaid, we offer direct financial assistance through the Saint Joseph’s Health Fund.
